Member Rights & Responsibilities Statement – a statement notifying you of your rights and responsibilities as a patient and Co-Op member. This statement is found in your policy document by clicking on the link above, and is also available online at: https://mountainhealth.coop/member-rights-and-responsibilities/
Prescription Drug Benefits Formulary – Provides information on prescription medications covered under your plan. You may search any drug on our formulary located on our website https://mountainhealth.coop/pharmacy/; click on the “Members” tab at the top of the page, then select “Pharmacy.”
Provider Directory – Provides information on the providers within our network. If you visit an out-of-network provider, your costs will be higher than if you utilize an in-network provider. Provider information can be found on our website https://mountainhealth.coop/find-a-doctor/; or click on the “Find a Doctor” button on the top of our website (https://mountainhealth.coop).
Member ID Card – You should receive your ID Card within 7-10 business days. You can also access a digital copy of your Member ID card by logging into your Member portal at https://mountainhealthcoop.vbagateway.com/ and selecting the “Member ID Card” option on the left of your home screen.
Making A Payment – You may submit your premium payment via the following:
- Setting up auto-payments can be done through options 1 and 2 below.
- Option 1- Call us at 800-299-6080 to get to automated payments or to speak with one of our Member Service representatives.
- Option 2- Log onto our website, www.mountainhealth.coop, select the “bill pay” at the top of the screen, and log in.
- Option 3– Pay by check. Please make check payable to MHC and include your billing account number. Mail check to:

Language Services & Accessibility – We have free interpreter services to answer any questions you may have about our health plans or drug coverage. To get an interpreter, call us at 801-213-4008. Someone who speaks your language can help you. This is a free service. If you need your plan information in a different format, such as large print or braille, please call 800-299-6080.
