Vision Exam Reimbursement

Did you know that your eyes can be a key indicator of your overall health?Ā 

This is why Mountain Health CO-OP has added a vision exam reimbursement to your medical plan and we have worked to keep it as simple as possible.Ā 

Here's how it works.

Helpful Hint:
Advise members to check with their provider regarding the cost of the exam ahead of time to make the most of the annual reimbursement.

*This is a reimbursement program, which allows you to go to any vision provider. You are not required to show an insurance ID card because the provider does not bill us directly. Instead, you submit a request for reimbursement directly to the COOP.
The documentation should include:
-Proof that the covered service was received;
- Date of service;
- Name and address of provider (if available, provider NPI);
- Patient’s name, address, and member ID.

Member is not required to provide proof of payment to the CO-OP; only proof of services received.

Ideally, a printout/itemized statement from the licensed provider is best.
Submit Documentation:
Submit proof of services to the CO-OP. For convenience, we provide three ways to submit:

The CO-OP will reimburse you and your dependents listed on the policy up to $60 each for the eye exam.

Dental Exam & Cleaning Reimbursement

We will reimburse up to $100 for dental exams or cleanings.

Starting September 1st, 2021 Mountain Health CO-OP members who utilize Telehealth Services (non-covid related) will be charged a co-pay. Co-Pays vary based on your CO-OP plan. For the last year and a half the CO-OP has covered all telehealth services in full, all covid related health issues until June 1, 2022.

In-Home Health Assessment

Availability begins January 1st for members in Idaho, Montana, and Wyoming.

We’re offering FREE in-home* and telehealth assessments for members and paying $50 for members to complete them!

We’re working with Advantmed to offer this benefit and support your health.

Centers of Excellence Program

As a member-governed health insurance CO-OP, we value your overall health.  We also understand that costs associated with services and procedures can cause hardships – both financially and logistically.

We created the Centers of Excellence* (COE), giving you access to care at high-quality facilities with provisions to reduce your out-of-pocket costs.

*LINK and Access Care Idaho plans do not have access to the Centers of Excellence Program.

Here's how it works

Waived Deductible

Member deductible may be waived* for services provided at a COE.

*To qualify, you must receive prior authorization, which can be obtained by callingĀ 801-587-2851.

Travel Expenses

Travel expenses related to the member will only be reimbursed for care done at Mountain Health CO-OP’s approved Centers of Excellence (COE) and must be out of state or more than 60 miles from the patient’s residence.

Reimbursement
All travel-related reimbursement requires prior authorization, which can be obtained by calling 801-587-2851.

Travel Expense Reimbursement
Limited to $10,000 per calendar year per member.

Air Transportation
Arranged by and billed to Mountain Health CO-OP.

Transportation
Transportation provisions cover the member and one caregiver. Travel by personal vehicle will be reimbursed at the IRS standard mileage rate, round trip, from your home zip code to the zip code of the provider location.

Lodging

Lodging is arranged by the member then billed to Mountain Health CO-OP.

Daily Per Diem
Member and caregiver will be given a daily per diem equal to the current federal meals and incidental daily rate for the location in which the member will receive treatment.
