Description

Modifier 22 is defined by “CPT Manuel” as Increased procedural Services.

 

Key Purpose

For use when services performed are more extensive than the usual service included in the procedure described by CPT/HCPS code guidelines.

Appropriate Use

Only report with a procedural code that has a global period of 0, 10, 90 days.

Documentation Requirements

Modifier 22 requires the concise statement within the operative report to clearly identify additional work and the reason for the additional work.

Increased Intensity – Increase level of service

Time – Above and beyond the usual procedural time

Technical difficulty of procedure – Increased level or additional technique of the procedure

Severity of patient’s condition – Increased risk of the patient while in the procedure

Physical and mental effort required – Providers own mental and physical efforts with the procedure.

 

Example Scenario

A surgeon performs a cholecystectomy with lysis of adhesions for a patient who is morbidly obese.

Scar tissue from prior surgery and the patient’s overweight physical condition affects the procedure duration, so performing this takes much longer than usual.
