Included below is important information about your health insurance coverage and outlines exactly where to retrieve the information.
Member ID Card – Your member id cards are enclosed. You can also access a digital copy of your Member ID card by logging into your Member portal at https://mountainhealthcoop.vbagateway.com/ and selecting the “Member ID Card”
option on the left of your home screen.
Provider Directory – Provides information on the providers within our network. If you visit an out-of-network provider, your costs will be higher than if you utilize an in- network provider.

Prescription Drug Benefits Formulary – Provides information on prescription medications covered under your plan. You may search any drug on our formulary located on our website https://mountainhealth.coop/pharmacy/; click on the “Members” tab at the top of the page, then select “Pharmacy.”
Individual and Small Group Summary of Benefits and Coverage – A summary of your specific plan can be found on our website https://mountainhealth.coop/plans-listing/; select your state or scroll down for more options to search for your plan. You may also access your plan documents when signed into your member portal or request hard copy by emailing: [email protected]. *Large group members please contact your group leader.
Individual and Small Group Policy Document– a detailed document containing all of your policy information can be found on our website https://mountainhealth.coop/plans-listing/; select your state or scroll down for more options to search for your plan. Premium Payment Options-Individual Members Only- You may submit your premium payment with one of the below options. Auto pay is available using options 1 and 2.
- Option 1- Call us at 800-299-6080 to set up automated payments or to speak with one of our Member Service representatives.
- Option 2- Log into your portal on our website, select “Login” at the top right-hand side of screen. Once logged in, scroll down to the bottom right-hand side of screen, and select “Manage my Billing”.
- Option 3-Visit our guest payment portal
- Option 4– Pay by check. Please make check payable to MHC and include your billing account number. Mail check to:

Language Services & Accessibility – We have free interpreter services to answer any questions you may have about our health plans or drug coverage. To get an interpreter, call us at 800-299- 6080. Someone who speaks your language can help you. This is a free service. If you need your plan information in a different format, such as large print or braille, please call 800-299-6080. TTY may be accessed by calling 711 or TDD may be accessed toll free by calling 1-800-346- 4128.

Benefits may change from year to year. You will be notified of any changes and provided the opportunity to change plans prior to the annual Open Enrollment period.
Member Service– When you have questions about your claims, benefits, or would like a printed copy of any member documents, please feel free to call us at 800-299-6080 or send us a secure message through your Member Portal by logging in and choosing the “Message Center” option on the left-hand menu.
